HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

I. Our Commitment to Your Privacy

Pixel Ecom LLC, operating as Preva (“Preva,” “we,” or “our”), is required by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) to maintain the privacy of your Protected Health Information (“PHI”) and to provide you with this Notice of Privacy Practices (“Notice”). We are committed to safeguarding your PHI.

This Notice applies to all PHI created, received, or maintained by Preva in connection with the Services.

II. What is Protected Health Information (PHI)?

PHI includes any information that:

  • Relates to your past, present, or future physical or mental health or condition
  • Relates to the provision of healthcare to you
  • Relates to payment for healthcare services
  • Identifies you (or could reasonably be used to identify you)

Examples include: your name, contact information, medical history, weight, height, medication history, treatment plans, and payment records.

III. How We May Use and Disclose Your PHI

A. Uses and Disclosures That Do Not Require Your Authorization

Public Health ActivitiesReporting to public health authorities for disease prevention, FDA reporting, or product recalls.
Health Oversight ActivitiesDisclosing to government agencies for audits, investigations, and licensure.
Judicial and Administrative ProceedingsDisclosing in response to court orders, subpoenas, or other lawful process.
Law EnforcementDisclosing for law enforcement purposes as permitted by HIPAA.
Serious Threat to Health or SafetyDisclosing to prevent a serious threat to health or safety.
Workers’ CompensationDisclosing as authorized by workers’ compensation laws.

B. Uses and Disclosures Requiring Your Authorization

The following require your written authorization:

  • Marketing communications that involve payment from third parties
  • Sale of PHI (Preva does not sell PHI)
  • Any other use or disclosure not described in this Notice

You may revoke an authorization in writing at any time. Revocation does not affect uses or disclosures made before revocation.

IV. Your HIPAA Rights

Right to Request RestrictionsYou may request restrictions on how we use or disclose your PHI.Submit a written request. We are not required to agree to all restrictions.
Right to Request Confidential CommunicationsYou may request alternative means or locations for communications.Submit a written request specifying the preferred method.
Right to a Paper CopyYou may request a paper copy of this Notice at any time.Email privacy@trypreva.com or call customer support.
Right to Breach NotificationYou have the right to be notified if there is a breach of your unsecured PHI.We will notify you in writing within 60 days of discovery.

V. Our Responsibilities

We are required by law to:

  • Maintain the privacy and security of your PHI
  • Provide you with this Notice of our legal duties and privacy practices
  • Notify you following a breach of your unsecured PHI
  • Abide by the terms of this Notice currently in effect
  • Obtain your written authorization for uses and disclosures not described in this Notice

VI. Changes to This Notice

We reserve the right to change the terms of this Notice and to make the new Notice effective for all PHI we maintain. Material changes will be posted on our Site and communicated via email.
